Crescent Care Foundation (ABN 37 656 537 370) is an NDIS-registered, ACNC-registered not-for-profit organisation based in South Western Sydney. We are committed to delivering high-quality, person-centred disability support services to participants with complex and high-intensity needs. We hold registration across a range of NDIS support categories, including Specialist Support Coordination and high-intensity daily activities.
We are currently seeking an experienced and highly professional Female Support Worker to provide tailored, participant-centred support to one of our valued participants based in Annandale.
About the Role
This is a specialised support worker position for a single participant – Brigitte, a 54-year-old woman living with multiple complex psychosocial and mental health diagnoses, including:
- Schizophrenia
- Bipolar Disorder
- Post-Traumatic Stress Disorder (PTSD)
- Anxiety Disorder
- Depression
- Multiple comorbid psychosocial conditions
The successful applicant will provide regular, structured support to assist Brigitte with her daily living activities, community participation, and overall wellbeing. The participant requires a calm, consistent, and professional support presence from someone who deeply understands the nature of complex mental health conditions.
Key Responsibilities
- Provide consistent, structured, and participant-focused daily living support
- Assist with personal care, domestic tasks, and community access as outlined in the support plan
- Implement strategies aligned with the participant's NDIS plan, support plan, and any relevant behaviour support plan
- Maintain clear and accurate shift notes, incident reports, and daily documentation
- Communicate regularly and professionally with the Support Coordinator
- Support the participant to build independence, confidence, and daily living skills
- Respond calmly and appropriately during periods of mental health distress
- Uphold the participant's dignity, privacy, and rights at all times
- Attend team meetings and mandatory training as required
Essential Requirements
Gender
- Female – this is a genuine occupational requirement under the Anti-Discrimination Act 1977 (NSW) in light of the participant's personal circumstances, diagnosis, and expressed preferences
Experience
- Minimum 1 year of demonstrated experience supporting individuals with complex psychosocial and/or mental health conditions
- Specific experience with schizophrenia, bipolar disorder, PTSD, anxiety, and/or depression highly regarded
- Demonstrated experience maintaining professional boundaries in a support worker or healthcare context
- Experience working within NDIS-funded supports preferred
English Language Proficiency – Non-Negotiable
Excellent and fluent English communication is a strict requirement of this position.
The successful candidate must demonstrate:
- Excellent spoken English – clear, articulate, and easy to understand
- Strong written English – ability to write accurate, professional shift notes and reports
- Strong reading comprehension – ability to read and understand support plans, behaviour support plans, and NDIS documentation

Qualifications & Compliance
- Certificate III or IV in Individual Support, Mental Health, Community Services, or a related discipline (minimum)
- Current NDIS Worker Screening Check (or willingness to obtain prior to commencement)
- Current Working with Children Check (WWC)
- Current First Aid Certificate (HLTAID011) and CPR
- Valid Australian driver's licence and reliable insured vehicle
- Current COVID-19 vaccination in accordance with NSW Health guidelines
- Right to work in Australia
Desirable Attributes
- Mature-aged applicant, preferably 45 years of age or older
- Lived understanding or professional exposure to the complexities of psychosocial disability
- Additional training in trauma-informed care, mental health first aid, or de-escalation
- Experience working under a Behaviour Support Plan
- Familiarity with NDIS Quality and Safeguards Commission standards
- Stable, consistent, and reliable availability
