Support Coordinator (NDIS)

Hours: Minimum 32 hours per week (days and hours negotiable)

We work alongside participants, families and service providers to deliver personalised, high-quality Support Coordination that empowers people to achieve their goals.
We're looking for an experienced and compassionate Support Coordinator to join our small, supportive team. Based in our beautiful office near the ocean, you'll enjoy a positive workplace where connection, flexibility and individuality are valued. While we enjoy a relaxed team culture, the work is fast-paced and often complex, so we're looking for someone who can confidently manage competing priorities while always putting participants first.
About the Role
You will:
- Support participants to understand and implement their NDIS plans.
- Coordinate a range of supports for participants with varying levels of complexity.
- Advocate for participants and assist them to overcome barriers.
- Build strong relationships with participants, families and service providers.
- Empower participants to build capacity, independence and confidence.
- Maintain accurate case notes, documentation and participant records.
- Meet key performance indicators (KPIs) while maintaining a high standard of service.
- Manage a caseload of 30+ existing clients
About You
You'll ideally have:
- A minimum of two years' experience as a Support Coordinator (preferred).
- A background in disability, mental health, community services, social work or a related field.
- Strong knowledge of the NDIS and person-centred practice.
- Experience managing complex participant needs and competing priorities.
- Excellent communication, organisation and problem-solving skills.
- Empathy, resilience and a genuine passion for supporting others.
- The ability to work independently while contributing positively to a close-knit team.
- Knowledge of Autism Spectrum Disorder (ASD) and other co-occurring conditions.
- Knowledge of local, mainstream and NDIS-funded service providers.
- A current Driver Licence, NDIS Worker Screening Check, Working With Children Check and National Police Check (or willingness to obtain where applicable).
