About the role
Temporary full-time / part-time opportunity to lead clinical pharmacy practice, support multidisciplinary care delivery, and drive continuous improvement in medicines management across STARS services.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
Facilitate medicines use evaluation, quality use of medicines projects and contribute to clinical research activities within the Pharmacy unit.
Provide accurate, timely and appropriate clinical information and implement liaison systems with hospital staff, General Practitioners, community pharmacies and residential care facilities, to optimise continuity of care.
Contribute to the development and maintenance of clinical practices, procedures and protocols that support the continuum of care incorporating an interdisciplinary focus as well as the standardisation and optimisation of digital systems.
