- Classification: Social Worker Grade 2 Year 1 - Year 4 (SC21 - SC24) or Community Development Worker Class 2B Year 1 - Year 7 (XX7 - ON15)* plus super ( Enrolled Nurses can be credentialed as CDW)
- Salary range: Social Worker $90,235.60 - $103,724.40 (annual full time salary), Community Development Worker $83,434.40 - $100,204 (annual full time salary)

Join Bayside Health’s Support at Home team, supporting older adults to live independently with choice and dignity. As a Care Partner, you’ll coordinate personalised care, complete assessments, develop care plans, connect clients with services, and support them to manage their care budgets.
Working in a collaborative team, you’ll advocate for clients, monitor wellbeing, and deliver responsive, person-centred care aligned with aged care standards while contributing to continuous improvement and positive outcomes.
