- Support young people to build independence, recovery and community connection
About the Role
Barwon Health is launching a new Youth Prevention and Recovery Care (YPARC) service, an innovative 10-bed sub-acute residential program supporting young people aged 16–25 years as an alternative to hospital admission or as a step-down from inpatient care.
YPARC delivers recovery-oriented, trauma-informed youth mental health care within a 24/7 supported, home-like environment focused on functional recovery, independence, wellbeing and meaningful community connection. The service brings together clinical, psychosocial rehabilitation and lived experience approaches to help young people build confidence, strengthen daily living skills and achieve their personal recovery goals.
As a Psycho-Social Rehabilitation Program Worker, you will play a key role in supporting young people to develop the skills, confidence and supports needed to live meaningful and connected lives. Working collaboratively within a multidisciplinary team, you will provide practical, emotional and psychosocial support through individual and group-based rehabilitation activities that promote recovery, independence and successful community participation.
Key Responsibilities
- Deliver recovery-focused rehabilitation support through individual and group programs that build daily living skills, social connection, confidence, wellbeing and community participation.
- Create safe and therapeutic environments by providing practical assistance, mentoring and emotional support that reflects trauma-informed, person-centred and recovery-oriented practice.
- Work collaboratively with consumers, families, carers and multidisciplinary teams to support goal setting, recovery planning and successful transition back into the community.
What We Are Looking For
We are seeking passionate and motivated professionals who are committed to supporting young people on their recovery journey. You will bring a genuine belief in recovery, strengths-based practice and the ability to build positive, therapeutic relationships with young people experiencing mental health challenges.
You will also demonstrate:
- Experience supporting people experiencing mental ill-health, psychosocial challenges or recovery needs.
- Knowledge of recovery-oriented and trauma-informed practice principles.
- Strong interpersonal, communication and relationship-building skills.
- Ability to facilitate individual and group-based rehabilitation activities.
- A collaborative approach to working within multidisciplinary teams.
- Commitment to promoting independence, social inclusion and community participation.
- Flexibility to work across a 24/7 residential service environment.
- Relevant qualification in Mental Health, Community Services, Social Work, Psychology, Occupational Therapy, Youth Work or a related field (desirable).
