We are seeking expressions of interest from a skilled and compassionate Community Nurse to join our growing clinical team as we expand into aged care home care services.
About the Role
As a Community Nurse, you will provide clinical care and support to clients in their homes, working closely with Registered Nurses and the broader care team to ensure safe, coordinated, and high-quality care.
You will play an important role in supporting clients’ health, wellbeing, and independence through ongoing assessment, clinical care, and education.
Key Responsibilities
- Deliver in-home clinical care including wound care, medication support, continence care, and chronic disease management
- Conduct initial and ongoing clinical assessments
- Monitor, document, and report changes in client health status to supervising clinical staff
- Complete risk assessments in the home environment to support safe care delivery
- Support post-hospital discharge care and recovery planning
- Provide education and guidance to clients and families regarding health conditions and care plans
- Maintain accurate, timely, and professional clinical documentation
- Collaborate with the wider care team to ensure coordinated, person-centred care
- Work in accordance with all relevant clinical standards, policies, and guidelines
What We’re Looking For
- Registered Nurse with current AHPRA registration
- Experience in aged care, community nursing, or acute care
- Strong clinical assessment and critical thinking skills
- Excellent communication and documentation abilities
- Ability to work independently in community settings
- Compassionate, respectful, and client-focused approach to care
- Commitment to person-centred and safe clinical practice
Requirements
- Current AHPRA registration as a Registered Nurse
- National Police Check (or willingness to obtain)
- Working rights in Australia
- Current First Aid and CPR certification
- Valid driver’s licence and reliable, insured vehicle
- Understanding of clinical governance and community care principles
