Your new role:
As a Senior Social Worker within the Pathways Team, you will provide advanced clinical social work services to patients experiencing complex discharge barriers and extended hospital stays across Princess Alexandra Hospital.
Key responsibilities include:
Deliver expert social work assessment, intervention and complex case management for long stay inpatients across acute and subacute services.
Lead and support complex discharge planning involving aged care, disability services, guardianship and community supports.
Provide specialist advice and consultation to multidisciplinary teams to address barriers to discharge and improve patient flow.
Contribute to service improvement, quality initiatives and the development of evidence-based clinical practices and pathways.
Support and mentor junior clinicians and students through supervision, education and professional guidance.
Collaborate with internal and external stakeholders to achieve safe, patient-centred outcomes for complex patients.
About you:
You are an experienced and compassionate Social Worker who thrives in complex clinical environments and is passionate about improving outcomes for vulnerable patients. You bring strong clinical judgement, exceptional communication skills and a collaborative approach to navigating challenging discharge scenarios.
To be successful, you will have:
Demonstrated experience in complex discharge planning, long stay patient management and hospital-based social work practice.
Strong knowledge of aged care, disability services, guardianship legislation and community support systems including the NDIS.
Proven ability to work effectively within multidisciplinary teams and provide expert clinical advice to stakeholders.
Experience supervising, mentoring or supporting less experienced clinicians and students.
A commitment to service improvement, evidence-based practice and delivering exceptional patient-centred care.

About us:
The Pathways Team is an interprofessional (IP) team of Allied Health Professionals with the goal to address the top three barriers to long stay patient discharges identified in the long stay census: access to Residential Aged Care beds, along with barriers pertaining to NDIS and legal matters. The team of Social Workers, Psychologists, Occupational Therapists, Physiotherapists, and HP1s work to facilitate discharge of complex long stay patients.
The team aims to achieve the following outcomes across MSH:
Reduced risk of patient clinical and functional decline and hospital acquired complications associated with extended length of stay.
Direct positive impact on ED, QAS ramping and DRG length of stay KPIs.
Increased efficiency with complex discharge planning.
