About the role
The Family Based Care Practitioner delivers comprehensive, trauma-informed and culturally safe case management across kinship and foster care services, supporting children, young people, carers, and families throughout the care journey. Working in partnership with Child Safety and community services, the role promotes safe, stable, and culturally responsive placements while upholding legislative requirements and the Aboriginal and Torres Strait Islander Placement Principle. This is a full time permanent role, based at Burleigh.
What you'll be doing
- Deliver Connecting Kin, Equipping Kin, Supporting Kin and Supporting Foster Care functions.
- Undertake kin connection work, family mapping and engagement.
- Prepare and equip kin for placement, including capability building and readiness assessments.
- Provide ongoing case management, placement support and cultural connection.
- Support foster carers with training, retention and placement stability.
- Maintain accurate case notes, reporting and compliance documentation.
- Work collaboratively with Child Safety, internal teams and community partners.
- Participate in risk assessment, safety planning and crisis response as required.

Selection criteria
- Relevant qualifications are highly regarded
- Demonstrated ability to work independently and as part of a team
- Experience working with the community sector and in a range of partnerships
Requirements
- A Blue Card for working with children ('No Card No Start' policy applies)
- National police check
- Current drivers' license.
- LCS2 clearance withing 3 months of commencing employment (this is a risk-assessed role under the Child Protection Act 1999)
- IDENTIFIED POSITION - To perform this role it is essential that the person who holds the position be an Australian Aboriginal or Torres Strait Islander person. It is therefore a genuine occupational requirement under section 25 of the Anti-Discrimination Act 1991 that applicants are Australian Aboriginal or Torres Strait Islanders. A Confirmation of Aboriginality will need to be provided.
