Community Services #1, in partnership with Your Side Australia, is seeking a Care Coordinator (non-clinical) to support clients transitioning from hospital to home through the Southern and Murrumbidgee NSW Local Health District Out of Hospital Care (OHC) Program. This role supports service delivery across Southern & Murrumbidgee NSW, with travel to Head Office in Canberra in line with operational needs.

About the role:
- Undertake goal-directed, strengths-based assessments (telephone and face-to-face) and identify client needs, risks and supports required for safe discharge and community living.
- Develop and coordinate short-term, tailored care and service plans aligned to OHC guidelines and approved package budgets.
- Coordinate referrals and service delivery with clients, carers, providers and NSW Health referrers; act as a key point of contact for client-related matters.

About you:
- Certificate IV (or equivalent) in Community Services, Health Services or similar, with at least 2 years’ relevant experience.
- Demonstrated experience delivering assessment, referral and support services to individuals and families with diverse cultural, emotional and social needs.
- Proven experience in care coordination, including planning, monitoring and adjusting supports, and managing client exits.
What we offer:

Requirements:
Current driver’s licence and ability to obtain/maintain required pre-employment checks and certifications (including National Police Check, Working with Children Check and First Aid), plus mandatory training as required.
