The Community Chronic Care Program Allied Health Team Leader is responsible to the Stream Leader Integrated Care and Chronic Disease for the operational management of Allied Health staff in the Community Chronic Care Program. The Community Chronic Care Allied Health Team Leader will ensure the provision of high-quality interdisciplinary services that are consistent with ISLHDs CORE Values through leadership, strategic direction and operational management of service delivery, staff and effective resource management.

ISLHD operate four hospital sites in the Illawarra: Coledale District Hospital, Bulli District Hospital, Wollongong Hospital and Shellharbour Hospital.
