We are seeking an experienced and passionate Aged Care and NDIS Client Acquisition Assistant to join our growing team in Footscray. This role is ideal for someone who is committed to delivering high-quality, ethical support and making a genuine impact in participants and consumers lives.
About the Role
As a Client Acquisition Assistant in Home Care and NDIS, you will be the key person supporting prospective clients from their first enquiry through to the commencement of services. You will assess client needs, prepare tailored budgets, and ensure each client is supported to access services that align with their goals, preferences and circumstances.
You will play a central role in the growth of Empower Plus Care by showcasing our point of difference, recommending personalised care options, and partnering with internal and external stakeholders to deliver a seamless onboarding experience.
This role requires someone who is a confident communicator and relationship builder with a passion for supporting older people. You understand aged care pathways and enjoy guiding clients with empathy and clarity.
Key Responsibilities
- Use initiative to source and secure new opportunities
- Follow up enquiries from prospective participants and consumer
- Ensure a smooth transition from initial enquiry to commencement of care
- Prepare service agreements, other agreements as required in a timely manner
- Take ownership of new client intake and working with various external stakeholders until client is fully transitioned into the business.
- Build trust with prospective clients, families and advocates using a consumer directed approach
- Conduct comprehensive assessments of client needs and goals
- Complete accurate, timely documentation and provide handover to Care Partner
- Maintain strong relationships with internal teams and external partners
- Promote Empower Plus Care services to clients, professionals and communities
- Manage all marketing and promotions including leads from internal and external sources
- Develop and manage strong referral pipelines through Support Coordinators, Plan Managers, hospitals, and community organizations
- Ad-hoc administration duties as requested by management
- Fulfil other duties as required by management and other department personnel as requested/required
- Prepare monthly reports as directed by the Management
Requirements
- Certificate of Service Coordination, Community Services or similar qualification or significant experience in client acquisition within the disability or aged care sector
- Experience delivering excellent customer service
- Comprehensive understanding of aged care & NDIS funded programs/services and supports to help accurately inform new clients.
- Confidence and competency in using business programs, technology and electronic documentation systems
- Highly organised with strong attention to detail
- Confident is discussing, presenting and recommending services to new clients
- Organisational and time management skills
- Current driver's licence and willing to travel in the community
