Join the Transitional Aged Care Program (TACP) and make a meaningful difference supporting older adults as they regain independence and achieve their personal goals following a hospital stay. We currently have two Allied Health Assistant positions available, one temporary full‑time and one permanent full‑time, with the permanent position designated for targeted recruitment in line with our commitment to building a workforce that reflects the community we serve. As part of a unique, client‑centred reablement program, you will deliver therapy and care in both community and residential settings across Bankstown, Fairfield and Carramar. Working under the guidance of Allied Health Professionals and Team Leaders, you will support clients through home exercise programs, self‑care retraining, community access and transport to appointments while contributing to a multidisciplinary team focused on improving health outcomes and quality of life for older people across South Western Sydney.
Are You the Right Fit?
You are enthusiastic, reliable and passionate about working with older people in a community or residential setting. You enjoy working both independently and collaboratively within a multidisciplinary team and bring strong communication and problem‑solving skills to your role. You are confident following direction from Allied Health Professionals while delivering high‑quality, person‑centred care to clients and their families. You take pride in supporting therapy programs and contributing to the smooth running of the service through both clinical and administrative tasks. You are adaptable and motivated to work across a diverse and growing community, where you can make a real difference in people’s lives.
