Client Intake & Onboarding
- Manage enquiries from initial contact through to service commencement
- Coordinate onboarding, service agreements, and documentation
- Liaise with participants, families, and Support Coordinators
- Ensure compliance with NDIS Quality Standards
- Maintain accurate intake and referral records
Service Coordination Support
- Act as a key point of contact for participants and families
- Conduct regular check-ins to ensure service satisfaction
- Work closely with internal teams to ensure continuity of care
- Escalate risks or concerns where required
Stakeholder & Business Development
- Build and maintain relationships with Support Coordinators, Plan Managers, hospitals, and allied health professionals
- Develop and grow referral networks across Melbourne
- Attend networking events and promote Cosy Home Care services
- Maintain an up-to-date referral partner database
Marketing & Brand Growth
- Support local marketing initiatives and outreach
- Promote Cosy Home Care services across Melbourne
- Assist in increasing brand awareness and market presence
About You:
Essential Requirements
- Diploma or Degree in Community Services, Allied Health, or related field
- Minimum 3 years’ experience in intake, client liaison, or support coordination
- Strong knowledge of NDIS Practice Standards & Quality Indicators
- Experience in disability, direct care, or aged care services
- Proficiency in CRM systems and Microsoft Office
Compliance Requirements
- NDIS Worker Screening Check
- Police Check
- Working With Children Check
- Valid Australian Driver’s Licence

Pay: $75,000.00 – $78,000.00 per year
