Registered Nurse – Community Care
About the Role
We are seeking a Registered Nurse to join our growing mobile healthcare team across Sydney. You will provide high-quality nursing care to clients in their homes, supporting independence, wellbeing, and positive health outcomes. Our client base includes aged care, disability, rehabilitation, and Department of Veterans' Affairs (DVA) clients.
Key Responsibilities
- Conduct clinical assessments and develop individualized care plans.
- Deliver and coordinate nursing services in the community.
- Monitor client progress and review care plans as required.
- Liaise with GPs, allied health professionals, support coordinators, families, and DVA stakeholders.
- Maintain accurate clinical documentation and ensure compliance with relevant standards.
- Promote client-centred care and positive health outcomes.

About You
- ✅Current AHPRA registration as a Registered Nurse.
- ✅Experience in community, aged care, disability, rehabilitation, chronic disease management, or DVA services is desirable.
- ✅Strong clinical assessment, communication, and care planning skills.
- ✅Ability to work independently and collaboratively within a multidisciplinary team.
- ✅Current driver's licence and reliable vehicle.

Pay: $87.00 – $110.00 per hour
