- Deliver patient-centred care through virtual inpatient and community-based models, supporting safe and effective treatment in the home
- Work within a collaborative Ambulatory Care / HITH team providing innovative alternatives to traditional hospital admission
- Collaborate closely with community nursing services, Emergency Department, and inpatient teams to support timely and coordinated patient discharge
- Develop and apply a broad range of clinical skills across home, clinic, and virtual care environments
- Contribute to multidisciplinary care planning and coordination to support optimal patient outcomes and continuity of care
