We are seeking a compassionate and reliable Support Worker to provide high-quality care and support to clients in their homes and/or community settings. The successful candidate will assist individuals to maintain their independence, achieve their goals, and participate in daily activities.
Key Responsibilities
- Provide personal care assistance, including showering, dressing, grooming, and toileting as required.
- Assist clients with daily living activities, meal preparation, and household tasks.
- Support clients to attend appointments, social activities, and community outings.
- Follow individual support plans and document client progress accurately.
- Promote client independence, dignity, choice, and wellbeing.
- Administer or assist with medications (if qualified and authorized).
- Maintain professional relationships with clients, families, and healthcare professionals.
- Report incidents, concerns, or changes in client wellbeing promptly.
Requirements
- Certificate III or IV in Individual Support, Disability, Aged Care, or a related field (preferred).
- Previous experience in disability support, aged care, or community services (preferred).
- Current First Aid and CPR certification.
- National Police Check and Working With Children Check .
- Valid driver's licence and reliable vehicle.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
