Right at Home Adelaide Central is seeking an experienced and compassionate Registered Nurse / Clinical Care Partner to join our growing team.
This is an exciting opportunity for a clinically skilled nurse who is passionate about supporting older Australians to remain independent, safe, and well within their own homes.
As a Clinical Care Partner, you will play a key role in clinical assessments, care planning, quality assurance, client reviews, staff mentoring, and supporting the delivery of exceptional community-based care under the Support at Home Program, Home Care Packages, and private services.
About the Role
You will work closely with clients, families, care workers, allied health professionals, and external stakeholders to ensure services are clinically appropriate, person-centred, and aligned with Aged Care Quality Standards.
This position combines clinical leadership, client relationship management, care coordination, and quality oversight within a supportive multidisciplinary team.
Key Responsibilities Clinical Assessment & Care Planning
- Conduct comprehensive clinical and care needs assessments in clients' homes.
- Develop, implement, and review individualised care plans.
- Identify clinical risks and implement appropriate interventions.
- Support reablement, restorative care, and wellness-focused approaches.
- Monitor client outcomes and recommend service adjustments as required.
Clinical Leadership
- Provide clinical guidance and support to care workers and support staff.
- Assist with delegation and supervision of care activities.
- Conduct competency assessments and staff observations.
- Support medication management processes and clinical governance activities.
- Promote evidence-based practice and continuous improvement.
Client & Family Support
- Build strong relationships with clients, families, advocates, and representatives.
- Support clients in achieving their goals and maintaining independence.
- Respond to enquiries, concerns, incidents, and complaints professionally and promptly.
- Participate in case conferences and multidisciplinary reviews.
Quality, Compliance & Governance
- Ensure services comply with:
- Aged Care Quality Standards
- Support at Home Program requirements
- Clinical Care Standards
- Infection Prevention and Control requirements
- Organisational policies and procedures
- Maintain accurate and timely clinical documentation.
- Participate in incident investigations, risk management, and quality improvement initiatives.
- Support audit preparation and compliance monitoring activities.
Care Coordination Support
- Collaborate with rostering and scheduling teams to ensure services align with assessed client needs.
- Assist with service reviews and care package utilisation.
- Support onboarding of new clients and service commencement activities.
After-Hours Support
- Participate in a rotating clinical on-call roster (where applicable).

About You
You are an experienced Registered Nurse who is passionate about community-based care and helping older Australians live independently with dignity and choice.
Essential Requirements
- Current AHPRA Registration as a Registered Nurse.
- Minimum 2-3 years' post-registration nursing experience.
- Experience in Aged Care, Community Care, Home Care Packages, Support at Home, Disability, Rehabilitation, or Chronic Disease Management.
- Strong clinical assessment and care planning skills.
- Excellent communication and relationship-building abilities.
- Strong organisational and time-management skills.
- Ability to work independently and make sound clinical decisions.
- Current National Police Check.
- Current First Aid and CPR Certificate.
- Current Driver's Licence and reliable vehicle.
- Comprehensive Vehicle Insurance.
- Full Australian Working Rights.
Desirable
- Experience with Support at Home and Home Care Packages.
- Knowledge of Aged Care Quality Standards.
- Experience using VisualCare or similar client management systems.
- Experience in clinical leadership, mentoring, or case management.
- Wound management, continence, dementia, palliative care, or chronic disease management experience.

Pay: $80,000.00 – $90,000.00 per year
