About the role
As a Clinical Coordinator with RMH@Home Acute, you are pivotal to the multidisciplinary healthcare team, striving to deliver excellent care to our patients and their families.
You will provide clinical leadership and operational coordination across the service, supporting the delivery of acute hospital-level care in patients' homes.
Working closely with the Nurse Unit Manager, medical team and multidisciplinary colleagues, you will coordinate patient care, support the on-road nursing team, and help ensure the service delivers safe, timely, effective and person-centred care.
Your interactions with patients, families and colleagues will be guided by our values of People First, Lead with Kindness and Excellence Together , and our Clinical Governance framework, STEP: Safe, Timely, Effective, Person-centred Care .
Key Accountabilities
Provide senior clinical leadership and operational support across the service.
Act as the primary escalation point for the on-road nursing team.
Coordinate daily clinical operations to support safe, efficient and responsive care.
Collaborate with the Manager, medical team and Clinical Development Nurse to achieve optimal patient outcomes.
Foster a positive team culture through coaching, communication and shared problem-solving.
Promote excellence in patient care through quality improvement and clinical governance.
About You
You have at least 4 years acute hospital experience with a collaborative working style
Previous ANUM experience highly desirable
You have effective communication skills and a passion for continued professional development
Advanced negotiating and interpersonal skills
Demonstrated autonomous practice with efficient time management skills
