Home Caring is a reputable and fast-growing provider of NDIS and Aged Care services, nationwide. We are currently seeking a passionate and experienced Case Manager to join our team and support a growing number of clients in the Hurstville/ Rockdale/ St George areas.
This role is ideal for someone who is community-minded, culturally sensitive, and committed to providing high-quality, person-centred care. You’ll be working with clients across both Aged Care and Disability sectors who need support workers that are able to understand their first language and communicate their personal care needs, behavioural support plans and cultural preferences. This requirement for a second language is a genuine occupational qualification for this role, inline with our participants rights to "dignity, choice and control" is not intended to exclude or disadvantage applicants but ensure that we align the skill sets of support workers with the needs of our participants.
Key Responsibilities:
- Conducting client assessments and developing person-centred care/support plans
- Coordinating services, monitoring outcomes, and reviewing care plans regularly
- Advocating for clients and ensuring their cultural, linguistic, and health needs are met
- Supporting clients to access appropriate services and resources
- Liaising with families, support workers, and health professionals
- Ensuring compliance with the NDIS & Aged Care quality standards
- Enrolling and management of Home Care Packages for Aged Care clients and NDIS funding for Disability clients
What We’re Looking For:
- Must speak either Mandarin/Cantonese /Vietnamese and English
- Relevant HCP & NDIS Case Management experience
- Experience in Aged care, disability, or community health settings
- Strong communication, problem-solving, and organisational skills
- Empathy, cultural awareness, and a passion for helping others
- Current First Aid & CPR, National Police Check, and NDIS Worker Screening
- Driver's Licence and reliable vehicle
- Full working rights in Australia
- Ability to assist the growth of the business by sourcing and signing up clients
