This innovative service supports people living with complex and persistent mental illness to achieve their recovery goals and live well in the community.
Our service has recently expanded to operate 7 days a week, giving you the opportunity to work within a contemporary roster that improves continuity of care and supports consumers when they need us most.

What you'll be doing
In this rewarding role, you will deliver evidence-based, recovery-oriented care to consumers in the community, helping improve independence, wellbeing and quality of life.
Provide comprehensive mental health assessments, treatment and assertive outreach support
Develop and implement individualised care plans in partnership with consumers and carers
Build strong relationships with families, NGOs and healthcare providers
Advocate for consumers and facilitate access to services and community resources
Participate in clinical reviews, quality improvement initiatives and service development within our new 7-day model

Essential Criteria
Qualification: Relevant tertiary qualifications as a Registered Nurse
Registration: Demonstrated full registration with AHPRA as a Registered Nurse
Selection Criteria
Demonstrated skills in mental health and discipline specific assessments and interventions and experience working with people with severe and persistent mental illness.
Demonstrated understanding and ability to work within experience working in a team and/or knowledge of the principles that guide service delivery within this model of practice
Demonstrated ability to collaborate meaningfully with consumers and carers with a commitment and understanding of recovery principles in mental health service provision.
Demonstrated working knowledge of NSW Mental Health Act, 2007.
Demonstrated excellent communication skills including verbal, written, and good interpersonal skills.
Computer competence and literacy including the use of database systems.
Current NSW Driver’s License.
