This role provides an exciting opportunity for a Child and Family Health Nurse to deliver evidence-based, family-centred care that supports the health, development and wellbeing of children aged 0–5 years and their families. Working across home visiting and clinic settings, you will partner with families to assess needs, develop tailored care plans and coordinate care that empowers parents and carers to give their children the best possible start in life.
A key focus of this role is supporting vulnerable children and families, including those experiencing complex health, social, emotional or environmental challenges. You will play a critical role in identifying risk early, providing targeted support, advocating for families, and connecting them with the services and resources needed to improve outcomes and reduce inequities. Through a strengths-based and family partnership approach, you will help build resilience, enhance parenting capacity and support families to navigate periods of vulnerability and change.
As a valued member of a multidisciplinary team, you will provide culturally safe and compassionate care, ensuring families receive the right support at the right time. Your work will have a direct impact on child health outcomes, early intervention, parenting confidence and family wellbeing, particularly for those with complex needs or who may face barriers to accessing healthcare and support services.By working alongside families during some of their most critical and challenging life stages, you will help create positive and lasting impacts for children, families and future generations.
