The Transitional Aged Care Program (TACP) offers the chance to make a meaningful difference in the lives of older adults by supporting their journey toward reablement, independence and personal goal achievement. As an Registered Nurse, you will play a vital role in delivering nursing care to clients within their own homes in the Macarthur area. Your responsibilities will include completing holistic nursing assessments, wound care, pressure injury assessment & treatment. Client education including but not limited to continence, diabetes, medication management. You will contribute to a unique, client‑focused program while providing high‑quality clinical services that optimise health outcomes for clients and their families. You will also support the smooth operation of the service by completing essential departmental and administrative tasks.
