At Nightingale Home Care (NHC), we are on the lookout for a qualified and experienced Community Worker to join our expanding team. We provide quality in-home and community access support to NDIS and Aged Care participants across Geelong, the Surf Coast, and the Bellarine Peninsula.
At this time, NHC is seeking a Community Worker who is an 'all-rounder' and is passionate for providing support to clients with in-home personal care and support with accessing the community.

KEY RESPONSIBILITIES. Providing direct support to clients, while organising all the paperwork, application forms, andreferrals for specialists depending on their needs..
Responding to enquiries and intake for new NDIS clients as required.. Delivering individualized and tailored case management support in line with each programand its specific needs..
Working with local service providers, to create connections that could facilitate the clients'living skills and inclusion into the community..
Keeping a clear communication with the family members of the clients and support workersto follow up the improvements and achievements of their goals, with monthly home visits..
Conducting regular plan spend conversations, where the clients can understand their NDISplan and learn about the management of funding for supports..
Recognizing the clients' needs for services in the areas of accommodation, wellness, health,employment, and development in the community..
Working with internal and external staff & other Aboriginal services for all Aboriginal familiesregarding cultural connection and identity..
Working across other support services programs as required providing support at times ofcrisis and when required..
Providing a safe environment for vulnerable or at-risk people and reporting abuse.
Licence/Certification:
- Certificate III or IV in Individual Support, Ageing Support, Disability, Community Services or related qualification
- Minimum 1 year experience as a Community Worker or similar role
- Driver Licence (Required)
- First Aid and CPR Certification (Required)
- Working with Children Check (Required)
- NDIS Worker Screening Check (Required)
- National Police Certificate (Required)

Pay: $77,000.00 – $80,000.00 per year
