Community Nurse (Registered Nurse / Enrolled Nurse)Position Overview
We are seeking compassionate and skilled Registered Nurses (RNs) and Enrolled Nurses (EENs) to join our community nursing team. You will provide high-quality, person-centred nursing care to clients in their own homes and community settings, supporting their health, independence, and quality of life.
Key Responsibilities
- Provide nursing care to clients within the community and home care environment
- Complete client assessments and contribute to individualised care plans
- Provide clinical care including wound management, medication support, health monitoring, and chronic disease management
- Administer medications and treatments within scope of practice
- Monitor clients’ health conditions and escalate concerns appropriately
- Support clients with maintaining independence and achieving health goals
- Communicate effectively with clients, families, carers, GPs, and other healthcare professionals
- Complete accurate clinical documentation, progress notes, and reports
- Follow infection control, workplace health and safety, and organisational policies
- Participate in care coordination and multidisciplinary team collaboration
Essential Requirements
- Current AHPRA registration as a Registered Nurse (RN) or Enrolled Nurse (EEN)
- Experience in community nursing, aged care, disability, rehabilitation, or hospital settings (preferred)
- Strong clinical skills and ability to work independently
- Excellent communication and documentation skills
- Current Police Check and relevant screening requirements
- Current driver’s licence and reliable vehicle
