1. Support Worker – Lurnea, NSW, 2170 (Up to 20hrs/week) Female Client | Daily Care Assistance (Friday 9am-5pm or 8am-4pm & Saturday 10:00 am - 3:00pm)
2. Support Worker – Oran Park, NSW, 2570 (Up to 15hrs/week) Male Client | Daily Care Assistance (Thursday 9am - 2pm & Friday 9am - 2pm)
3. Support Worker –West Hoxton, NSW, 2171 (Up to 15hrs/week) Male Client | Daily Care Assistance (Thursday 9am - 2pm & Friday 9am - 2pm)

Seed Disability Services (NDIS Registered Provider) is seeking a reliable and compassionate Disability Support Worker to support a participant with community access and daily wellbeing supports.
Key Duties:
Support attendance at GP and health appointments
Assist with community outings, events, and appointments of the participant’s choosing
Provide daily wellbeing check-ins (non-clinical) and report concerns appropriately
Assist with light domestic tasks
Deliver supports one-on-one and in small group settings
Complete NDIS-compliant shift notes
Requirements:
- Updated First Aid & CPR Certificate
- Updated Police Check
- Updated Driver’s License
- NDIS Worker Screening Check
- Working With Children Check
- NDIS Worker Orientation Module Certificate (“Quality, Safety and You”)
- Manual Handling Certificate
- Updated Infection Control Certificate
- COVID-19 Infection Control Training (if available)
- Updated Vehicle Registration & Insurance
