Mosaic Multicultural Connections is seeking an experienced and compassionate Registered Nurse to join our Aged Care team for a short-term contract, working 3 days per week.
This role has become available to support a period of increased clinical demand and offers the opportunity to make a meaningful impact by providing high-quality clinical care to older people living independently in their own homes. While initially offered as a short-term position, there is potential for the role to develop into a longer-term opportunity.
As our Registered Nurse, you will provide clinical expertise and support to ensure our clients receive safe, person-centred care that promotes independence, wellbeing and quality of life.
Key responsibilities include:
- Conducting comprehensive clinical assessments.
- Performing wound assessments and wound management.
- Catheter management and related clinical care.
- Continence assessments and clinical recommendations.
- Post-fall assessments and clinical reviews.
- Developing and reviewing care plans in collaboration with clients and the broader care team.
- Providing clinical advice and guidance to support workers and coordinators.
- Maintaining accurate clinical documentation and ensuring compliance with relevant legislation, standards and organisational policies.
- Liaising with clients, families, GPs, allied health professionals and other healthcare providers to coordinate quality care.
To be successful in this role, you will have:
- Current registration as a Registered Nurse with AHPRA.
- Demonstrated experience in community, aged care or primary healthcare nursing.
- Experience in wound care, continence management and clinical assessment.
- Strong assessment, clinical reasoning and documentation skills.
- Excellent communication and relationship-building abilities.
- A commitment to delivering person-centred care with compassion and professionalism.
- Current Australian Driver’s Licence and willingness to travel between client homes.
- Current National Police Check (or willingness to obtain).
- Current First Aid and CPR (or willingness to obtain).
