About the Role
We are seeking an experienced and motivated Service Coordinator to join our growing team based at our Caringbah Head Office.
This role supports NDIS participants living throughout the Sutherland Shire and surrounding suburbs, ensuring they receive high-quality, person-centred services that help them achieve their goals and maintain their independence.
This is a casual position working 2 days per week, with the potential to transition into a permanent role following a successful probationary period.
Key Responsibilities
- Coordinate supports and services for NDIS participants in line with their individual goals and needs.
- Develop and maintain strong relationships with participants, families, support workers, allied health professionals, and other stakeholders.
- Conduct participant reviews and monitor service delivery to ensure positive outcomes.
- Assist with onboarding new participants and act as a key point of contact.
- Maintain accurate and compliant participant records and case notes.
- Liaise with internal teams to ensure effective scheduling and service coordination.
- Identify and resolve service-related issues in a timely and professional manner.
- Support participants to access community, mainstream, and funded supports.
- Ensure services are delivered in accordance with NDIS Practice Standards and organisational policies.
About You
To be successful in this role, you will have:
- A minimum of 2 years' experience in NDIS Service Coordination, Support Coordination, Case Management, or a similar role.
- Experience working with NDIS participants and a strong understanding of the NDIS framework.
- Excellent communication, relationship-building, and problem-solving skills.
- Strong organisational skills with the ability to manage competing priorities.
- High attention to detail and experience maintaining accurate records.
- Proficiency with Microsoft Office and client management systems.
- The ability to work independently while contributing positively to a collaborative team environment.
Applicants with Level 2 or Level 3 Support Coordination experience are encouraged to apply.
Essential Requirements
- Current NDIS Worker Screening Check.
- Current Working With Children Check.
- First Aid & CPR
- Current Driver Licence and reliable vehicle.
- National Police Check (or willingness to obtain).
- Right to work in Australia.
