We are specifically looking for experienced Disability Support Workers, particularly those with a background in supporting individuals with high behaviours and mental health challenges. Responsibilities of the role

Support clients in leading an active lifestyle and engaging with their community through person-centred programs and meaningful activities.
- Build strong, positive relationships with people from diverse backgrounds, gender expressions, personalities, and cultures.
- Utilise positive behaviour strategies, a person-centred approach, and a trauma-informed lens to provide effective support.
- Assist clients with personal care, hygiene, meal preparation, medication management, community access, and appointments.
- Maintain accurate progress notes and ensure thorough record-keeping.
- Demonstrate strong computer, written, and verbal communication skills.
- Apply transferable skills in providing care, empathy, and support to others.
- Respectfully assist with personal care and domestic duties, ensuring dignity and independence.

IMPORTANT CONSIDERATION - VAN SIZE
Chosen Family is seeking a female Support Worker for a participant who requires transportation in a large wheelchair. Due to the size of the participant's wheelchair, applicants must have a mid-to-large-sized SUV or van with sufficient boot space to safely accommodate the wheelchair.
Please note that previous support workers had vehicles that were unfortunately not large enough to accommodate the participant's wheelchair. To avoid this issue, we kindly ask interested applicants to provide the following:

- A photo of the vehicle with the boot open, clearly showing the available cargo space.
