About the Role

We have an exciting opportunity for an experienced Grade 2 Social Worker to join the Social Work Service at Casey Hospital in a full-time, ongoing position.
Working within a fast-paced acute hospital, you'll support patients and families experiencing complex health and psychosocial challenges, helping them navigate their healthcare journey through compassionate, evidence-informed practice. Every day brings something different, with opportunities to work alongside medical, nursing and allied health professionals to achieve the best possible outcomes for patients.
This role offers exposure to a broad range of acute inpatient clinical presentations, making it an excellent opportunity to further develop your clinical reasoning, crisis intervention, discharge planning and multidisciplinary practice while working within a supportive and experienced team.
In this role, you will:
Complete comprehensive psychosocial assessments within an acute hospital setting.
Provide counselling, advocacy and practical support to patients and their families.
Coordinate complex discharge planning and facilitate access to community services.
Work collaboratively within multidisciplinary teams to deliver person-centred care.
Support patients experiencing complex social, family and health-related challenges.
Contribute to quality improvement, service development, research and education initiatives.
Participate in student supervision and support the ongoing development of the Social Work profession.

About You
You are an experienced and compassionate Social Worker who enjoys working in a dynamic acute hospital environment and is committed to delivering high-quality, person-centred care.
You will also bring:
Eligibility for membership with the Australian Association of Social Workers (AASW).
Demonstrated experience within a hospital or other complex healthcare environment.
Strong psychosocial assessment, crisis intervention and clinical reasoning skills.
Experience managing complex discharge planning and care coordination.
Excellent communication, collaboration and stakeholder engagement skills.
A commitment to evidence-informed practice and continuous professional development.
The ability to build strong relationships with patients, families and multidisciplinary teams while supporting positive health outcomes.
