The role
As one of our Care Partner's - CHSP, you'll hold a caseload of clients and be their central point of contact, assessing needs, coordinating services, conducting reviews, managing referrals through My Aged Care, and making sure each client receives safe, well-planned, person-centred support.
This is a coordination and relationship management role. You'll work alongside our client liaison team, support workers, schedulers and clinical team, but you won't be delivering personal care or domestic assistance yourself. Your value is in your judgement, your knowledge of the aged care system, and your ability to keep clients and services running smoothly.
What you'll be responsible for

Managing a caseload of CHSP clients as their primary point of contact

Assessing client needs and developing, monitoring and reviewing care plans

Coordinating services and managing referrals via My Aged Care

Ensuring accurate records & care notes

Working closely with clients, and their families
-
Maintaining compliance with CHSP guidelines and the Aged Care Quality Standards
Who we're looking for - experience is essential
This is not an entry-level or Support Worker position. To be successful you'll bring:

Demonstrated experience in aged care, community care or case management/coordination

Sound working knowledge of CHSP funding (and ideally the Support at Home reform)

Familiarity with My Aged Care, and care management systems (we use Nightingale)

Strong organisation, prioritisation and documentation skills

Confident communication with clients, families and a multidisciplinary team

A relevant qualification in aged care, community services, nursing or allied health (or equivalent experience) would be highly regarded
What you'll need before starting

National Police Clearance / aged care worker screening

The right to work in Australia

A current WA driver's licence
