About the Role
We are seeking an experienced, highly observant, and empathetic Home Care Worker to provide comprehensive in-home care for a client residing in Hope Island.
The client lives with a complex health profile, including Dementia, Type 2 Diabetes, Chronic Fatigue, and High Blood Pressure. Due to a high risk of dizziness and oedema, this role requires a carer who is deeply vigilant about physical safety, fall prevention, and skin integrity, while providing sensitive, reassuring support for the client's depression and anxiety.
Key Responsibilities
Cognitive & Emotional Support: Providing gentle, dementia-specific care. Managing symptoms of anxiety and depression with a calm, reassuring presence, predictable routines, and validation techniques.
Mobility & Fall Prevention: Due to frequent dizziness, providing steady, close-proximity supervision and physical assistance during transfers and walking to prevent falls.
Clinical Condition Management:
Monitoring for signs of diabetic fatigue or blood sugar fluctuations.
Assisting with the management of lower-limb oedema (e.g., assisting with compression garments, ensuring legs are elevated when resting, and monitoring skin condition).
Continence Care: Providing discreet, dignified, and hygienic assistance with urinary incontinence, ensuring skin integrity is maintained.
Healthy Lifestyle Support: Preparing diabetes-friendly meals and encouraging gentle, safe movement as guided by health professionals.
Domestic Assistance: Light housekeeping to maintain a clutter-free, safe environment to eliminate tripping hazards.
About You
You are a seasoned care professional who looks at the "whole person," not just a checklist of tasks. You possess excellent clinical observation skills, noticing subtle changes in a client's physical status (such as increased dizziness or swelling) or emotional state, and you know how to report these changes accurately.
Requirements & Qualifications
Minimum Certificate III in Individual Support, Ageing Support, Disability, or a student nurse looking for community work.
Proven experience working with clients living with Dementia and complex chronic health conditions (Diabetes, Hypertension, Oedema).
Strong understanding of fall prevention strategies and dignity-first continence care.
Current NDIS Worker Screening Check
National Police Check.
Current First Aid & CPR Certificates.
Valid Australian Driver’s Licence with a reliable, insured vehicle.
