The Specialist Palliative Care Service is based on the Long Jetty Health Care campus, and requires a permanent part time dynamic and motivated Clinical Nurse Specialist Grade 2 (CNS to work across the Central Coast.
About The Opportunity
We are a small multi-disciplinary team who works collaboratively to support patients and families across the Central Coast.
The CNS2 role is required to provide clinical care across different locations, including with hospital patients referred to our consultation service, working in Intake/Triage or based in the community seeing people at home or in RACFs.
The role works with other multidisciplinary team members to support clients and their families through to End of Life care and death. The position is varied and interesting and requires a good knowledge of Microsoft Word, Excel, Outlook and eMR. Solid communication skills, relevant clinical experience and strong problem-solving abilities will contribute to the position’s success. A solid individualised orientation program is provided for new Team members, where you will learn all aspects of the role.
A current Australian driver's license is a prerequisite.
Work hours are 0830 to 1700 Monday to Friday as well as weekend and on-call shifts which are covered by most staff on a rotation basis.

About You
We are looking for someone who has:

Holds current AHPRA registration as a Division 1 Registered Nurse with post-registration qualifications and at least 3 years’ experience in Palliative Care or relevant specialty.

Demonstrates advanced clinical skills, leadership, and the ability to provide complex care.

Communicates effectively both verbally and in writing and works collaboratively within a multidisciplinary team.

Has a current Australian Driver’s licence.

Holds a National Criminal Record Check and Working With Children Check.
