ROLE PURPOSE
· The Care Partner works in partnership with consumers of Government funded aged care supports, through the Support at Home program (CHSP and HCP) to coordinate high quality care and support services through internal and external providers in order to achieve the best outcomes for the consumer’s quality of life.
· The Care Partner is responsible for the safety and quality of their own professional practice, consistent with ITSOWEL’s policies and procedures, Government program guidelines and Aged Care Quality Standards.
POSITION TASKS AND RESPONSIBILITIES
The employee’s responsibilities include but are not limited to:
Sustainability
· Monitor client home care budgets and expenditure, ensuring services are provided within funding levels of each client
· Work with accounts and finance team to ensure accurate and timely financial administration and reporting
· Meet care management target hours each month.
Administrative
· Complete case notes after all consumer visits and phone communication
· Develop, monitor and maintain consumer support plans with a consumer directed care service delivery framework
· Ensure that all services are delivered in accordance with consumer’s support plan which is supported by the MAC support plan and that it provides sufficient detail and guidance for staff to provide a comprehensive care service in line with the consumer’s goals and directions
· Maintain accurate records and reports in accordance with the Aged Care Quality Standards and the ACQC’s compliance policies.
· Follow all policies and procedures for clinical care management, risk management, consumer care plan reviews and other systems as required as per Government guidelines and internal policies and procedures.
· Maintain and manage associated providers for the provision of home care services in line with Government guidelines
· Identify and control work, health and safety issues in the consumer’s home
· Ensure all complaints from consumers are investigated and resolved in line with ITSOWEL’s policies and procedures
· Action annual feedback and survey forms including follow up of improvements required
· Update Feedback Register as required
Consumer Satisfaction and Impact
· Assist and enable clients to understand their aged care funding in accordance with financial budgets and Government guidelines
· Develop, monitor and maintain consumer support plans with a consumer directed care service delivery framework
· Ensure that all services are delivered in accordance with consumer’s support plan and that they provide sufficient detail and guidance for staff to provide a comprehensive care service in line with the consumer’s goals and directions
· Handling complaints and incident reports related to home care services
· Client Intake and Community Reach
· Follow up all MAC referrals promptly and provide an efficient, high level of customer service and helpful first point of contact for actual and potential clients and referrers
· Engage with consumers, carers, the community and service providers to improve access to ITSOWEL’s Support at Home program (HCP and CHSP)
· Work effectively with other aged care services team members to ensure effective internal referrals from CHSP to HCP
· Work with executive on marketing campaigns for Support at Home program (HCP and CHSP)
Education and training
· Participate in all training
· Demonstrate a willingness to continue to inform and educate yourself on all aspects of the Support at Home program and My Aged Care
· Be up to date with changes to Government policy
· Service planning and development
· Actively contribute to the development of quality improvement processes
· Actively contribute to service planning and improvement
· Participate in the preparation and completion of quality reviews and accreditations
· People and Culture
· Demonstrate teamwork and foster good communication with Care Support Workers and the whole ITSOWEL team
· Participate in information sessions, training, staff meetings, conferences, community forums and seminars as required
· Work collaboratively within the Company as well as with other external service providers and health professionals
Accountability
· The position is fully accountable for the quality, integrity and accuracy of the advice provided and work performed
· Ensure compliance with Aged Care Quality Standards, government policies and regulations, ITSOWEL policies and procedures.
SKILLS, KNOWLEDGE AND EXPERIENCE REQUIRED
Essential
a. Experience in a similar role, aged care environment, assessment or experience in allied health
b. Knowledge of the common physical, psychological and social issues affecting frail older people and people living with dementia
c. Demonstrated understanding of issues affecting older people from culturally diverse background
d. Demonstrated understanding of Australian Government funded aged care system
e. Demonstrated understanding of the Aged Care Quality Standards
f. Highly developed interpersonal and communication skills, including ability to undertake complex negotiations and develop professional relationships with internal and external stakeholders
g. Proven high level organisational and motivational skills
h. Ability to meet deadlines on a daily basis, manage competing priorities and tasks
i. Ability to participate in the quality improvement process
j. Commitment to working with people from culturally diverse backgrounds
k. Current comprehensive driver’s licence and vehicle with full comprehensive insurance
l. Valid Police Check
m. Working with Children Check.

Pay: $46.70 – $47.92 per hour
Expected hours: 35.0 per week
