An exciting opportunity exists for a motivated Diabetes Educator with a strong interest in and commitment to improving the health of people with type 2 diabetes to join the dynamic and supportive team at the Diabetes Centre, Royal North Shore Hospital.
The successful applicant would optimise and extend the interface between primary and tertiary care through the development and delivery of expert clinical assessment, referral and coordination of health and community services for people with diabetes with complex care requirements. The actions of the CNC are directed towards achieving improvements in clinical practice, standards of care and improved outcomes for people with type 2 diabetes and complex care requirements.

Candidates will need to meet the following criteria:
- Registered Nurse – with Australian Health Practitioner Regulation Agency ( AHPRA ) and a minimum 5 years full time post graduate experience.
- Experience in roles related to Integrated Care/Connecting Care or chronic disease management.
- Relevant post graduate qualification to the role.
- Demonstrated clinical expertise and clinical leadership abilities within the field of integrated care with ability to mentor others.
- Demonstrated excellence in written and verbal communication skills and computer literacy in Microsoft applications.
- Demonstrated ability to work well within a multidisciplinary team.
Ability to contribute to the development of clinical procedures, guidelines and policies.
