About the role:
Griffith Base Hospital are on the search for passionate pharmacists to join the Pharmacy team! You will assist in the provision of a comprehensive clinical pharmacy service in accordance with the SHPA Standards of Practice for Clinical Pharmacy Services and support medication safety practices and quality improvement initiatives in accordance with National Standard 4.
You will play a vital role as part of the multi-disciplinary team, involved in the delivery of education and information to other health care professionals along with providing medicines information to patients and carers.
As the Pharmacist, you will be responsible for:
Providing medication-related education to medical and nursing personnel, and to patients/carers
Providing discharge counselling to patients/carers to ensure a full understanding of the medications prescribed as well as any alterations to the medication regimen and provide whenever possible a discharge medication list.
Maintaining accurate documentation
Maintaining continuing professional education to fulfil the annual requirement for AHPRA Pharmacy Board registration

Our ideal candidate will demonstrate:
Current registration as a pharmacist authorised to practice by the Australian Healthcare Practitioner Agency (AHPRA).
Experience and skills in clinical pharmacy with the ability to apply this knowledge to practical situations. Post registration hospital pharmacy experience or post graduate relevant qualification (or working towards) will be highly regarded.
Sound judgement and demonstrate initiative and independence in problem solving and prioritizing workload.
Ability to work effectively as a member of a multidisciplinary team.
