Tasks & Responsibilities
- Assess clients’ individual needs, abilities, goals and support requirements and contribute to the planning, development and implementation of educational, training and support programs.
- Assist in developing practical strategies and routines that promote independence, daily living skills, social participation, communication and personal development.
- Implement individual support plans, behaviour support strategies, health-related plans, risk management strategies and other programs developed for participants.
- Support participants to develop and maintain skills relating to personal care, household responsibilities, meal preparation, communication, decision-making, community participation and other areas of daily living.
- Work collaboratively with participants and their support networks to identify changing needs and ensure supports remain appropriate and person-centred.
- Monitor and report on the progress of clients against their identified goals, support plans and individual programs.
- Observe changes in a participant’s behaviour, physical presentation, emotional wellbeing, functional capacity and support requirements and escalate concerns appropriately.
- Maintain accurate progress notes, documentation, records, incident reports and data collection as required by legislation, NDIS requirements and True Living policies and procedures.
- Provide relevant information and observations to management, families, Support Coordinators, Behaviour Support Practitioners, allied health professionals and other authorised stakeholders where appropriate.
- Participate in reviews of participant supports and contribute information regarding progress, emerging risks, changing needs and opportunities for further skill development.
- Community Needs & Resources
- Assess and identify community resources that may support participants in the areas of health, welfare, housing, employment, education, training, recreation and other community-based services.
- Assist participants to access appropriate community services, facilities, activities and opportunities in accordance with their individual goals and preferences.
- Support participants to develop confidence and independence when accessing their local community.
- Liaise with relevant community organisations, government bodies, NDIS representatives,
- Support Coordinators, Social Workers, allied health professionals and other service providers where required.
- Support individuals and their families by providing appropriate information, education and practical assistance relating to the participant’s daily support needs.
- Provide care and support to people with disability living within adult service units and group housing environments.
- Work collaboratively with families and representatives while maintaining the participant’s rights, privacy, dignity, choice and control.
- Support participants experiencing social, emotional or financial challenges by providing practical assistance, guidance and appropriate referrals or escalation to relevant professionals.
- Assist participants to build and maintain positive relationships with family, peers, housemates and members of the community.
The successful will need to have flexible availability, including weekends, evenings, public holidays, and overnight shifts.

Mandatory Requirements:
- Qualification Level Required: Diploma in Home and Community Care, Aged Care, Community Services, Individual Support, Disability, Mental Health or similar relevant field of study;
- At least one year full-time or equivalent Industry experience
- COVID Certificate
- NSW Valid driver’s license, Reliable Vehicle & Comprehensive Car Insurance
- First Aid certificate & CPR certificate
- Current Working with Children Check for paid roles (or willing to obtain) & Current Police check (or willing to obtain)
- NDIS Modules (or willing to obtain)
- A smartphone that has access to the internet and the ability to download rostering app
- Proof of vaccination
Pay: $40.00 – $45.00 per hour
Expected hours: No more than 38.0 per week
