Registered Nurse (RN) – Community Nursing

Nursing Centred Care Australia (NCCA) is a growing community healthcare provider delivering high-quality nursing and support services to NDIS participants, DVA clients, and older Australians. We are seeking a motivated and experienced Registered Nurse who is passionate about providing person-centred care and is looking to grow into a future leadership role.
About the Role
As a Registered Nurse, you will play a key role in delivering clinical services, coordinating supports, and ensuring high standards of care across our community clients.
Key Responsibilities
- Coordinate Supported Independent Living (SIL) clinical supports.
- Conduct clinical visits, including:
- Wound assessment and management
- PEG management
- Male and female catheter management
- Other community nursing interventions
- Complete continence assessments and prepare comprehensive reports.
- Undertake delegation assessments and develop delegation reports.
- Investigate and close clinical incident reports.
- Conduct new client intake assessments.
- Complete comprehensive client assessments.
- Develop and review person-centred care plans.
- Provide clinical guidance and support to support workers and enrolled nurses.
- Maintain accurate and timely clinical documentation.
- Work collaboratively with participants, families, support coordinators, and allied health professionals.
Essential Requirements
- Current AHPRA registration as a Registered Nurse.
- Current National Police Check.
- Current NDIS Worker Screening Clearance.
- Current Working with Children Check (or willingness to obtain).
- Current First Aid and CPR certificates.
- Current Australian Driver's Licence and reliable vehicle.
- Excellent clinical assessment, communication, and documentation skills.
- Ability to work independently while contributing to a supportive team environment.
Desirable
- Experience in community nursing.
- Experience working with NDIS and/or DVA clients.
- Experience with wound care, PEG management, continence care, and catheter management.
- Experience preparing clinical assessments and professional reports.
