Your Care is currently seeking experienced Disability and Aged Care Support Workers to join our growing team in Albury-Wodonga and surrounding areas. We are looking for compassionate, skilled, and dedicated individuals who are committed to empowering people to live independently, participate in their communities, and achieve the outcomes that matter most to them.
As a Support Worker with Your Care, you will support a diverse range of participants, including those with complex disabilities, psychosocial disabilities, mental health challenges, and participants requiring high-intensity supports. We are seeking team members who can provide safe, respectful, and person-centred support while promoting independence, capacity building, and community inclusion.

What the Role Involves
As a Support Worker, you will assist participants to live independently and achieve their goals while delivering high-quality, person-centred support. This may include:
- Personal care and activities of daily living
- Community access, social participation, and capacity-building activities
- Supporting participants to develop independent living skills
- Transporting participants to appointments, activities, education, and employment opportunities
- Supporting participants with psychosocial disabilities and mental health challenges
- Delivering support in accordance with Positive Behaviour Support Plans where applicable
- Supporting participants with complex care needs and high-intensity support requirements
- Assisting with mealtime management, medication support, and other delegated supports in line with training and organisational policies
- Building meaningful and respectful relationships with participants, families, and allied health professionals
- Promoting participant choice, dignity, rights, and independence in all aspects of support
- Maintaining accurate documentation and reporting in line with NDIS Practice Standards

What We Are Looking For
To be successful in this role, you will have:
- Minimum Certificate III in Individual Support (Disability, Aged Care, Mental Health, or equivalent)
- Demonstrated experience supporting people with disabilities, psychosocial disabilities, mental health challenges, and/or complex support needs
- Experience working with participants requiring high-intensity supports (desirable)
- A strong understanding of person-centred practice, participant choice and control, and the NDIS Practice Standards
- The ability to implement support plans, behaviour support strategies, and risk management plans
- Excellent communication, problem-solving, and interpersonal skills
- The ability to remain calm, professional, and adaptable in complex situations
- Reliability, professionalism, and a commitment to participant outcomes
- A current Driver's Licence
- A reliable and registered motor vehicle

- NDIS Worker Screening Check

Pay: $40.00 – $60.00 per hour
