The Team & Role
Peninsula Care Group is committed to supporting individuals to make informed decisions about their future healthcare through Advance Care Planning (ACP) and access to Voluntary Assisted Dying (VAD) services. We are seeking an experienced and passionate Advance Care Planning & Voluntary Assisted Dying Team Leader to lead and support these important programs across Peninsula Health.
In this leadership role, you will provide operational and professional leadership to the Advance Care Planning and Voluntary Assisted Dying team, fostering a high-quality, person-centred service. You will drive awareness, education and engagement across the organisation and broader community, ensuring patients, families and clinicians are supported to navigate complex healthcare decisions with confidence and compassion.
Working collaboratively with multidisciplinary teams, community stakeholders and healthcare providers, you will promote the uptake of Advance Care Planning and support access to Voluntary Assisted Dying services in accordance with legislative and organisational requirements. You will play a key role in ensuring individuals with chronic and complex conditions, life-limiting illnesses, and older people have opportunities to document their healthcare preferences, appoint decision-makers, and receive care that aligns with their values and wishes.

As the Team Leader you will be expected to:
- Leading and supporting the Advance Care Planning (ACP) and Voluntary Assisted Dying (VAD) team to deliver high-quality, person-centred services.
- Promoting the profile, awareness and benefits of Advance Care Planning and Voluntary Assisted Dying services across Peninsula Care goup and the broader community.
- Building and maintaining strong relationships with clinical services, community organisations and key stakeholders.
- Supporting individuals and their families to navigate Advance Care Planning, including the appointment of Medical Treatment Decision Makers and Appointed Support Persons.
- Facilitating and supporting the completion of Advance Care Directives, Instructional Directives and related ACP documentation.
- Providing expert guidance and support to clinicians regarding ACP and VAD processes, legislation and best practice.
- Coordinating service delivery to ensure access to Voluntary Assisted Dying services in accordance with legislative and organisational requirements.
- Leading education, quality improvement initiatives and service development activities to enhance ACP and VAD services across Peninsula Care Group.
- Monitoring service performance, fostering continuous improvement and contributing to strategic planning and reporting requirements.

Skills & Experience Required
Allied Health Practitioner with current registration with Australian Health Practitioner Regulation Agency (APHRA) or eligible for registration with the relevant professional organisation (if Social Work).
OR General registration as a Registered Nurse with Australian Health Practitioner Registration Agency.
Sound understanding of Advance Care Planning aims and underlying principles.
Experience in palliative care, end-of-life care, Advance Care Planning, and or Voluntary Assisted Dying.
Current Victorian Driver’s Licence.
Aged Care Police Check.
Working with Children Check.
