The Role
In this position, you will deliver high‑quality, client‑centred Occupational Therapy services to community clients in their own homes across the South Metropolitan region (from Mandurah through to south of the river in Perth). This role offers a high degree of autonomy, supported by strong clinical leadership, clear scope and a well‑established multidisciplinary team.
The role includes a strong emphasis on:
- Assessment, prescription and reporting for Assistive Technology and Home Modifications (AT‑HM)
- Supporting clients to remain safe and independent at home through practical, goal‑directed OT interventions
- Working within a wellness and reablement framework aligned to community aged care programs
You will work as part of a multidisciplinary team (MDT), collaborating closely with physiotherapists, therapy assistants, community nursing staff, case managers, and the wider Bethanie team to deliver coordinated, person‑centred care.
You will also be supported by a Senior Allied Health Team Lead (Occupational Therapist) who provides clinical mentoring, guidance and professional support, ensuring you are not working in isolation.
As this is a community‑based role, you will be required to use your own reliable vehicle, with travel reimbursed in line with organisational policy.
Key Responsibilities
- Deliver Occupational Therapy assessments, interventions and programs for community clients
- Assess, prescribe and document Assistive Technology and Home Modifications (AT‑HM)
- Develop clear, goal‑directed care plans to support safety, independence and functional outcomes
- Produce high‑quality clinical documentation and reports to support funding and service delivery
- Work collaboratively within the multidisciplinary community care team
- Contribute to clinical quality, safety and continuous improvement initiatives
- Provide mentoring and support to relevant staff as required
Essential Criteria
- Tertiary qualification in Occupational Therapy and current AHPRA registration
- Current ‘C’ or ‘CA’ class driver’s licence
- Access to a reliable vehicle and willingness to travel across the South Metro region for client home visits
- Demonstrated experience in assessment, intervention planning and evaluation for community clients
- Strong client‑centred practice using a wellness and reablement approach
- Well‑developed communication, organisational and documentation skills
- Ability to work independently while contributing effectively within a multidisciplinary team
Desirable Criteria
- Experience working in community aged care and/or home‑based service delivery environments
- Knowledge of assistive technology, home modifications and falls prevention
- Experience supporting clients with cognitive change, including dementia
- Confidence working across functional, environmental and safety‑based OT interventions
