Family Support Coordinator
We are seeking a compassionate, reliable and suitably qualified Family Support Coordinator to join our organisation on a full-time basis.
The Family Support Coordinator will provide high-quality, person-centred support to people with disabilities living in an NDIS-supported accommodation setting. The role involves working closely with participants, their families, carers, support workers and other service providers to identify support needs, coordinate appropriate services and promote independence, wellbeing and social inclusion.
Key Responsibilities
- Provide person-centred support to participants with disabilities and their families.
- Assess and identify the support needs of participants and their families.
- Assist with the development, implementation and review of individual support plans.
- Coordinate access to appropriate disability, community, health and social support services.
- Liaise with participants, families, carers, support workers and other relevant service providers.
- Monitor participants' progress and identify changes in their support needs.
- Support participants to achieve their individual goals and promote independence, dignity, choice and wellbeing.
- Facilitate community participation and social inclusion.
- Support families and carers by providing information and assistance in accessing appropriate services and resources.
- Respond appropriately to complex or changing support needs and escalate concerns where necessary.
- Maintain accurate client records, case notes, progress reports and incident documentation.
- Ensure services are delivered in accordance with relevant NDIS requirements, organisational policies and legal and ethical obligations.
Pay: $75,000.00 – $85,000.00 per year
