Casual Support Coordinator | WA
District 360 is a registered NDIS provider growing our Western Australia team. We are looking for experienced Support Coordinators to join us on a casual basis.
What you will be doing
- Help participants understand and implement their NDIS plans
- Source and coordinate services from community, mainstream, and funded providers
- Monitor plan budgets and ensure supports align with participant goals
- Build strong relationships with participants, families, and support networks
- Complete accurate case notes, service agreements, and progress reports
- Work within NDIS Practice Standards and the NDIS Code of Conduct
What you need
- Certificate IV in Disability, Community Services, or related field (degree preferred)
- Demonstrated NDIS Support Coordination experience
- Strong knowledge of plan budgets, support categories, and the participant portal
- Current valid driver's licence and reliable vehicle
- Current NDIS Worker Screening Check (WA) or willingness to obtain prior to start
- Current First Aid and CPR certificate
