We are seeking a Service Coordinator For Our Supported Independent Living (SIL) Homes to join our growing, dynamic team in a full-time, permanent role, providing coordination and support to participants living in SIL homes across both North and South of the River.
About the role
- Oversee and manage operations in our Supported independent living home.
- Provide leadership and supervision to a team of support workers.
- Effectively liaise with external stakeholders to ensure seamless coordination of support services.
- Monitor the delivery of supports to participants, ensuring alignment with individual goals.
- Develop and implement effective risk management strategies for participants
- Regular oversight and reporting of restrictive practices
- Actively coordinate, monitor and report on supports in accordance with the participant's plans
- Provide timely Reports as required by our Stakeholders
- Work one-on-one with clients with regular weekly visits to ensure their support needs are being met.
- Comply with NDIS rules and regulations.
About you
- Key Requirements:
- Passionate about the NDIS sector, with proven experience in enhancing and upskilling clients’ lives.
- Strong staff management and leadership experience, with the ability to bring out the best in others.
- Creative thinker who can turn complex information into visual and accessible formats for our SIL participants.
- Ability to manage SIL homes effectively, while developing innovative ways to improve clients’ daily living skills and social engagement.
- Understanding of restrictive practices and their appropriate implementation.
- Family-oriented approach with a genuine commitment to participant wellbeing and inclusion.
- Flexible and adaptable in responding to changing client needs.
- Highly developed written and verbal communication skills.
- Valid Driver’s License and access to a reliable vehicle.
