About the Team
The Integrated Child and Youth Mental Health Service (CYMHS) provide complex secondary and tertiary level health care. CYMHS offers specialised mental health services for families with children and young people (birth to 18 years) who experience severe and complex mental health problems, and where their needs cannot be met by other services. CYMHS provides a comprehensive, recovery-orientated mental healthcare service that aims to improve the mental health and wellbeing of children and young people and their carer networks. Recovery principles are embedded into service delivery, culture and practice, providing consumers and their families, in collaboration with other services, access and referral to a range of programs that will support and sustain recovery. CYMHS combines hospital and community-based facilities to provide free consultation, assessment and treatment of children and young people experiencing serious mental health disorders and problems with their carer network.
About the Role
The Nurse Navigator (Paediatric Transgender) is a Registered Nurse accountable for the provision of specialised knowledge and advanced clinical practices supporting patients and families within the state-wide service provided by QCGS. The role involves leading the coordination of integrated care of QCGS patients outside of the metro Brisbane area. Gender healthcare involves multidisciplinary care and this role encompasses existing models of care at CHQ and provides a nurse-led model of care for a sub set of this population with complex care and coordination needs.
The Nurse Navigator role focuses on patient-centred healthcare and aims to facilitate the patient journey through the complex health system state-wide. This role aims to lead and facilitate a safe and efficient patient journey across the care continuum through the reduction of service fragmentation, educating and empowering individuals and families and coordinating patient care. The Nurse Navigator role utilises advanced mental health clinical and leadership skills and knowledge to deliver a positive patient centred health experience for individuals.
The Nurse Navigator is accountable to provide safe, quality and cost-effective family centred paediatric nursing care that is consistent with contemporary standards.
The Nurse Navigator is the lead nursing role with QCGS, working closely with QCGS Coordinator/Team Leader to support colleagues/peers with clinical decisions as well as overseeing the management of medical care delivery through clinical practice within the outpatient department at Queensland Children's Hospital (QCH).
