ACCORD CARE is trusted in the provision of person-centered disability support services to NDIS participants. We provide Supported Independent Living, In-home care, Community Participation, Short-Term Accommodation and Community Nursing.
About the role
We are looking for team members in our supported independent living arrangements in Boondall to provide 24/7 support in the home and out in the community.
The support worker's role and responsibilities will include:

Supporting people with personal care and day-to-day activities in the home and out in the community

Providing positive behaviour support to our clients

Completing appropriate documentation through shift notes, incident reporting and data collection
-
Contribute to the team’s overall objective of providing the best outcomes for our clients
-
Work with your team leader to implement and review plans around daily service provision
-
Maintain a professional relationship with clients and adhere to the NDIS and company code of conduct
-
Above all, be responsive, reliable and accountable to our clients and your team

Qualification/Experience:

Minimum Certificate III in Individual Support/Aged Care

(or the ability to acquire an appropriate qualification)
