What you’ll be trusted with:
You will be the key operational link between the Service Manager and frontline support teams, ensuring that what is planned is delivered consistently, safely, and to a high standard across our SIL homes.
On-the-Ground Coordination & Operations
Supporting the Service Manager by coordinating day-to-day operations across SIL homes
Managing rosters, shift coverage, and workforce allocation to ensure continuity of care
Leading house meetings and maintaining clear communication across teams
Acting as the first point of escalation for operational matters within the homes
Participant Intake & Service Delivery
Coordinating participant intake, onboarding, and transitions into SIL
Supporting implementation of individualised support plans on the ground
Facilitating family conferences and maintaining strong stakeholder communication
Ensuring supports are delivered in line with participant goals, dignity, and independence
Quality, Compliance & Safeguarding
Ensuring effective implementation, monitoring, and staff compliance with Mealtime Management Plans (MMPs), including safe swallowing practices and escalation of risks
Overseeing incident reporting, investigation, and follow-up, ensuring actions are completed and lessons are embedded into practice
Conducting regular audits of care documentation, progress notes, and compliance records to identify gaps and drive continuous improvement
Leading the implementation of Restrictive Practices in accordance with NDIS Commission requirements, ensuring all practices are authorised, documented, and proportionate to participant needs
Coordinating behaviour support plan implementation, ensuring staff understand and consistently follow approved strategies
Managing Restrictive Practice reporting, including:
Timely NDIS Commission reporting (Reportable Incidents)
Maintaining accurate internal registers and documentation
Supporting reduction and elimination strategies in line with Positive Behaviour Support frameworks
Supporting the Service Manager to ensure all services align with NDIS Practice Standards, Quality Indicators, and organisational policies
Team Support & Development
Delivering new staff orientation and induction at site level
Supporting staff performance reviews and day-to-day supervision
Assisting in implementing staff training and educational programs
Providing ongoing guidance to ensure consistent, high-quality support delivery
Who This Role Is Best Suited For:
This position is designed for an experienced professional who brings a strong foundation in disability service delivery and operational coordination within the NDIS environment.
The ideal candidate will demonstrate:
Proven experience in independently managing Supported Independent Living (SIL) homes
Sound knowledge of NDIS compliance requirements, including restrictive practices and associated reporting obligations
Strong capability in roster coordination, workforce management, and handling operational escalations
Experience in guiding and supporting teams to deliver consistent, high-quality care outcomes
The ability to effectively manage both people and documentation, maintaining high standards across service delivery and compliance
What you’ll need:
Proven experience as a Care Coordinator / Team Leader / SIL Manager for one year or more
Strong understanding of NDIS frameworks and reporting requirements
Experience with restrictive practice implementation & reporting
Excellent leadership, communication, and organisational skills
Current driver’s licence & NDIS Worker Screening
Working With Children check and Police Check
