We are now seeking a Service Delivery Coordinator (Intake & Review) to join our Care Services team in a permanent full-time capacity. In this role, you will be the warm and reassuring first point of contact for clients and their families - helping them take their first steps with us and ensuring they feel supported throughout their journey.
What You’ll Be Doing
You’ll play a key role in shaping the client experience from the very beginning. As the first point of contact for many clients and families, you’ll provide clear information, timely follow-ups, and compassionate guidance to ensure people feel confident and supported.
Key responsibilities include:
- Following up on referrals and care plan reviews within required timeframes
- Checking in on client progress, incidents, and safety assessments to support wellbeing and compliance
- Providing accurate advice on care plans, service options, and associated costs so families feel confident in their choices
- Handling calls and call-backs with professionalism, empathy, and understanding
- Responding to emails within a 24–48 hour timeframe
- Supporting smooth, respectful client onboarding experiences
- Working closely with the Intake & Review team to ensure every client feels supported from their very first contact
What You’ll Bring
We’re looking for someone who brings both capability and compassion - highly organised, proactive, and committed to delivering great outcomes for clients.
You will have:
- Minimum Certificate III in Aged Care (or equivalent)
- Certificate IV in Ageing Support or Community Services (highly regarded)
- Statutory Declaration
- Current National Police Check
- Current First Aid and CPR certificates
- Driver’s Licence
- Experience in administration, intake, scheduling, or a fast-paced service environment
- A genuine passion for supporting health and wellbeing
- Experience within community services (preferred)
- Familiarity with the My Aged Care Portal (highly regarded)
