About the opportunity:
Join our ever-growing team at SACARE as an Intake Coordinator today! We are currently on the lookout for an Intake Coordinator to work either Full Time across our accommodation and community services.
Based at our head office in Broadview, the successful candidate will be responsible for:

Managing daily enquiries and service requests within appropriate turn-around times and with a person-centred approach.

Vacancy, pipeline, and waitlist management.

Daily monitoring of the referrals inbox.

Collating and gathering relevant documentation from participants and other relevant stakeholders, to determine eligibility to receive SACARE’s services.

Accurate data management ensuring participant privacy and confidentiality.

Maintaining accurate records in a Client Relationship Management System (CRM) for weekly reporting purposes.

Creating and facilitating the development/scheduling of:
- Service agreements
- Quotations
- Community visits with potential participants (face-to-face where possible)
- Initial Support Needs and Risk Assessments

Collaborating with SACARE’s NDIS Specialist, finance, client services, scheduling and service co-ordination teams, and supported accommodation managers to ensure a timely and effective transition period.

Participating in various information sessions with the community.

Attending various internal and external networking events, including exhibiting at expo’s for increased referral sources.

Stakeholder/referrer relationship management (support coordinators, plan managers, hospital discharge planners, social worker teams, other referrers)

Collaborating with the Business Development Manager and the Marketing team to develop targeted campaigns, including EDM’s, Social Media posts and networking activities to assist in increasing referrals.
To be considered for the role, candidates need to have:

Qualifications in Cert IV in Business and/or Community Services (or relevant demonstrated experience).

NDIS Workers Check and Working with Children Check (Mandatory)

Excellent administrative, time management and organisational skills.
-
Demonstrated experience working within the Disability sector (or Community Services sector), including knowledge of NDIS and DHS standards.

Understanding of up-to-date Disability related legislation, policy and processes.

Ability to work autonomously, set own priorities and work to deadlines.

Ability to work collaboratively and influence a range of internal and external stakeholders, including colleagues, participants, families/carers/guardians.

Demonstrated problem-solving abilities and high degree of initiative.

High degree of professionalism, optimism, and resilience.

Excellent interpersonal and communication skills with the ability to work with individuals from all socio-economic and cultural backgrounds

Current unrestricted Driver’s Licence (mandatory)
