Client Access and Service Coordinator
The opportunity
ou will play a key role in managing the intake, onboarding and coordination of services across Warrigal Home Services, ensuring a seamless and responsive client access experience. Acting as a central point of coordination, you will support clients from initial enquiry and referral through to service commencement and ongoing service adjustments.
This role requires someone who is highly organised, proactive and confident managing competing priorities. You will translate care plans into deliverable services, coordinate service setup, manage service changes, and work closely with rostering teams to ensure timely and efficient service delivery.
Working collaboratively with Operations Managers, Care Services Managers, Care Managers and Team Leaders, you will ensure clients receive appropriate, person-centred services aligned with their care plans, funding arrangements and organisational policies.
You will also coordinate Assistive Technology and Home Modifications (AT-HM), ensuring timely procurement, supplier engagement and installation in line with client needs. Your attention to detail, communication skills and ability to manage multiple stakeholders will contribute to positive client experiences, safety, and service excellence.
What you’ll bring
Essential:
- Relevant experience in community, in-home aged care, or customer service coordination environments
- Demonstrated experience working in client service coordination, intake or service delivery roles within health, aged care or community services
- Highly developed communication and relationship management skills, including telephone-based assessment and intake support
- Strong organisational and coordination skills with the ability to manage multiple tasks and competing priorities
- Strong problem-solving skills and ability to respond effectively to changing service requirements
- Understanding of person-centred care principles and safe service delivery
- Strong attention to detail and ability to maintain accurate records and documentation
- Ability to identify and escalate risks to ensure client safety and service compliance
- Demonstrated ability to work collaboratively with multidisciplinary teams
Desirable:
- Certificate III or IV in Aged Care, Community Services, or related field
- Experience working within home care, community services or aged care programs
- Knowledge of the strengthened Aged Care Quality Standards and relevant funding program requirements
- Experience coordinating services for CHSP, Veterans’ Home Care or similar programs
- Experience coordinating assistive technology or home modification services
