About the role
In this role you will deliver wellness and lifestyle services and activities that empower and support residents to engage in meaningful activities that improve quality of life and align with SCC’s “Better for life” model.
Key Responsibilities include but are not limited to;
Support and enable residents to maximize their independence, health, wellness, and overall quality of life.
Use best-practice tools to design and deliver high-quality lifestyle and exercise services, focusing on person-centered care, goal setting, emotional support, and health literacy.
Facilitate resident participation in both facility-based and broader community activities tailored to their interests.
Work within a multidisciplinary team to identify residents at risk of frailty or decline and develop tailored Recovery Pathways.
Conduct regular audits to ensure all residential lifestyle programs align with Southern Cross Care procedures.
Support and guide staff, volunteers, and students in delivering meaningful projects and lifestyle activities.
Implement central Wellness and Lifestyle team projects designed to enhance client outcomes.

About you
We are looking for people who will be the difference in the lives of our residents and clients.
The successful candidate will have the following skills and experience;
Hold a minimum Diploma-level qualification in health promotion, social work, dementia care, health coaching, healthy ageing, or leisure and lifestyle.
Proven experience in conducting person-centered assessments, auditing, reporting, and managing wellness programs for older adults.
Skilled in the early identification of social withdrawal or physical decline, with experience in case management to improve outcomes.
A strong understanding of how to support residents living with dementia, including managing behaviors of concern.
Exceptional customer service skills with a track record of consulting regularly with residents and their families.
Strong relationship-building skills and the ability to balance a resident's right to independence and self-determination against risk management and duty of care.
Current Driver’s Licence, proficient keyboard skills, and experience using Google Suite and internal databases.
